On your first visit to Complete Eye Care we ask that you fill out and mail in our new patient forms that we will send to you. This will save time when you arrive for your appointment. Please be sure to bring all of the glasses you wear, all of your insurance cards, and a list of the medications and eye drops you are currently using. If you are transferring from another eye doctor, we will help you have your records transferred to our office. During the course of a normal eye exam we may need to dilate your pupils. For this reason, we suggest that you bring a pair of sunglasses to your appointment.
